Today, more than ever patients want to know what their options are when they have terminal illnesses and limited life expectancy. Our trained staff can guide patients through hospice care when no further treatment is desired or beneficial
The Attending Physician is the patient’s primary care physician and works as a member of the hospice team
Medical Director - Certifies the attending physician’s prognosis, leads the hospice team in developing a plan of care, and provides consultation to the attending physician
Registered Nurses - Evaluate patient’s condition during regularly scheduled visits, coordinates plan of care, and teaches the family proper patient care techniques and reviews medications.
Social workers - Licensed medical social workers provide emotional support for the patient and family. They assist with handling end of life issues and help with advanced directives
Certified Home Health Aides - Provides personal care services designed to give comfort
Chaplains - Provides patient and family with spiritual support and works as a liaison with clergy of individuals choice.
Therapy Services - Physical, Occupational and Speech Therapy
